Open Enrollment

Each year during the open enrollment period you may change your benefits. This year the open enrollment period is May 14 to May 31.

Personal Choice 10/20/70/Select Drug 10/20/35/OptiChoiceVision) is the “core” plan . Employees enrolling in a plan other than the core plan will be subject to a mandatory payroll deduction (pretax) for premium difference (if applicable) and will have to complete a payroll deduction authorization form. Healthcare Enrollment Forms are due in Human Resources by June 1, 2007. Anyone not returning the form by that deadline will automatically be enrolled in the core plan.

The district offers the option of using pre-tax dollars to pay for unreimbursed medical and/or dependent care expenses. There is no minimum or maximumamount to put into a Medical Flexible Spending Account. The Dependent Care Account limit is $5,000. Anyone who wants to start or continue contributing to either or both of these Section 125 plans must return the completed enrollment application to Human Resources by June 1, 2007.

You may also make a change to or enroll in disability insurance. Reducing the elimination period or signing up for the coverage will depend on medical underwriting approval. Completed enrollment/change applications are due in Human Resources by June 1, 2007.

Changes to life insurance may be made as well. To eliminate imputed income tax, you may elect to reduce the maximum death benefit on the life insurance policy to $50,000 by filling out a life insurance change form. If you reduce your death benefit to $50,000, you may not increase it to your maximum without medical underwriting approval.
